The User Social Process (PSU) from SESARAM received the 1st prize for Digital Transformation at the Integrated Care Awards 2026, an event held on October 2nd in Elvas.

The project, developed by the Social Service and the IT and Technologies Unit of SESARAM, was recognized in the ‘Digital Transformation’ category, which honors initiatives using digital technologies to optimize care delivery.
Implemented in 2016, the PSU replaced handwritten records with a unique digital process, transversal to all health units in the Autonomous Region of Madeira. This tool provides healthcare professionals with real-time access to relevant social information, promoting continuity of care and avoiding duplication.
The initiative contributes to a faster social intervention tailored to the user's needs, while also reducing administrative burden and enabling more efficient resource utilization. The project has been enhanced with new functionalities, such as Social Referencing and Clinical Discharge Monitoring.
This marks the second award for the SESARAM project at the Integrated Care Awards, having previously been recognized in the 2023 edition. The awards are organized by the PAFIC - Portuguese Association for Integrated Care.
