The project, which recognizes the case management model developed at ULSBE for the follow-up of people with complex chronic diseases, received the distinction on Friday in Elvas. In this model, a case manager accompanies each person's journey, coordinating the intervention of different professionals and care levels.
ULSBE's work was also selected to be included in the Handbook of Case Management, with a chapter dedicated to the institution's experience. The model was presented at the ICA meeting, which brought together various experiences in integrated care.
The award and inclusion in the Handbook are a recognition of the quality of the response developed by ULSBE's teams in case management and care integration.
