The Local Health Unit (ULS) of Barcelos/Esposende was awarded 1st Prize in the Organizational Transformation category of the 5th Integrated Care Award (ICA) 2026, for its "Integrar para Cuidar" project.

The project, which recognizes the case management model developed at ULSBE for the follow-up of people with complex chronic diseases, received the distinction on Friday in Elvas. In this model, a case manager accompanies each person's journey, coordinating the intervention of different professionals and care levels.
ULSBE's work was also selected to be included in the Handbook of Case Management, with a chapter dedicated to the institution's experience. The model was presented at the ICA meeting, which brought together various experiences in integrated care.
